  • Eric Carpenter, Danville, Ohio: 52-year-old killed after UTV leaves Harrisburg Road

Eric Carpenter, Danville, Ohio: 52-year-old killed after UTV leaves Harrisburg Road

Eric G. Carpenter, 52, of Howard, was killed Tuesday morning after the utility task vehicle he was driving left Harrisburg Road near Chapel Road in Jefferson Township, Knox County. The crash happened at about 4:44 a.m., when a motorist came across the wreck and called the Knox County 911 Dispatch Center. Carpenter was pronounced dead at the scene.

The Ohio State Highway Patrol’s Mount Gilead Post is investigating the crash. Troopers and deputies were sent to the area after the 911 call came in. The crash involved a 2024 Polaris RZR, which Carpenter was operating southbound on Harrisburg Road.

Investigators said the RZR traveled off the right side of the road. It then struck a fence and overturned. The vehicle eventually came to rest on its side in the roadway. Carpenter was ejected during the crash and suffered fatal injuries.

The crash left a 52-year-old Howard man dead before the day had barely begun. The early-morning wreck also brought emergency crews to a stretch of Harrisburg Road after the motorist discovered what had happened and alerted authorities.

At this point, the highway patrol has not released any additional details about what caused the RZR to leave the roadway. The investigation remains active as troopers work to establish the circumstances surrounding the crash. No other vehicle was identified in the patrol’s initial account.

Several local agencies helped at the scene. The Knox County Sheriff’s Office and the Eastern Knox County Joint Fire District assisted the highway patrol. The Mount Vernon Association of Police Chaplains and Danville Tire & Alignment also provided assistance following the crash.

For those who knew Carpenter, the details of the crash are only part of the story. He was a 52-year-old man from Howard whose life ended suddenly on a Knox County road. The investigation will now focus on what happened in the moments before the Polaris RZR left Harrisburg Road.

The crash remains under investigation by the Ohio State Highway Patrol’s Mount Gilead Post. Authorities have not announced any further findings or additional circumstances surrounding Carpenter’s death.

  • Theosco Middlebrooks, Seymour, Indiana: Northbound Jeep in southbound I-65 crash kills three

Theosco Middlebrooks, Seymour, Indiana: Northbound Jeep in southbound I-65 crash kills three

Theosco Middlebrooks was driving north on Interstate 65 early Sunday, but he was in the southbound lanes near Seymour. Minutes later, his Jeep collided head-on with a Ford Escape carrying 31-year-old Alyssa Osborn and her 1-year-old son, Waylon. All three people died from injuries suffered in the crash.

Indiana State Police said the collision was reported shortly after 2:30 a.m. Aug. 23 in Jackson County. The crash happened just south of the 48-mile marker, about one mile south of Seymour. A caller alerted authorities to the head-on collision in the southbound lanes.

Middlebrooks, 51, was from Louisville, Kentucky. He was driving a 2005 Jeep Liberty northbound in lanes meant for southbound traffic. Investigators said they did not yet know why the Jeep was traveling the wrong way when it struck Osborn’s vehicle.

Osborn, who lived in Scottsburg, was driving a 2023 Ford Escape southbound in the correct lanes. The Jeep and Ford collided head-on, leaving both drivers with fatal injuries. The Jackson County Coroner’s Office pronounced Middlebrooks and Osborn dead at the scene.

Osborn’s 1-year-old son, Waylon, was also inside the Ford Escape. Police said the child was properly restrained in the rear passenger seat. He suffered critical injuries and was taken to Norton Children’s Hospital in Louisville for treatment.

Waylon later died at the hospital from injuries suffered in the crash. His death brought the total number of people killed in the collision to three. What began as a wrong-way crash on a southern Indiana interstate ended with the deaths of two adults and a child.

Police believe alcohol consumption by Middlebrooks likely contributed to the crash. Toxicology testing was still pending, so investigators had not reached a final conclusion about the role alcohol played. The Indiana State Police Crash Reconstruction Team continued working to determine exactly how the Jeep entered the southbound lanes.

The southbound side of I-65 remained closed for about four hours while police investigated the scene and crews cleaned up the wreck. Seymour police, the Jackson County Sheriff’s Department, emergency medical personnel, Seymour Fire Department, Redding Fire Department, Vernon Township Fire Department and the Jackson County Coroner’s Office also assisted.

The investigation remains ongoing. At this stage, authorities have confirmed the direction of travel, the vehicles involved, the deaths of all three people and the pending toxicology results. The crash happened on Interstate 65 about one mile south of Seymour in Jackson County, Indiana.

Alyssa Osborn, Seymour, Indiana: Wrong-way crash kills Scottsburg mother and her 1-year-old son

Alyssa Osborn was driving south on Interstate 65 near Seymour early Sunday when a Jeep traveling the wrong way slammed into her Ford Escape. The crash killed the 31-year-old Scottsburg mother at the scene. Her 1-year-old son, Waylon Osborn, was badly hurt and later died at a Louisville children’s hospital.

The crash happened shortly after 2:30 a.m. on Aug. 23 in Jackson County, about one mile south of Seymour. A caller reported a head-on crash in the southbound lanes near the 48-mile marker. Indiana State Police sent troopers and its Crash Reconstruction Team to the scene.

Investigators said 51-year-old Theosco Middlebrooks of Louisville was driving a 2005 Jeep Liberty north in the southbound lanes. Police have not said why he was traveling against traffic. His Jeep hit Osborn’s 2023 Ford Escape head-on as she was traveling south in the proper lanes.

The impact was fatal for both drivers. Osborn and Middlebrooks suffered deadly injuries and were pronounced dead at the scene by the Jackson County Coroner’s Office. For Osborn’s family, the crash quickly became even more painful because her young son was also inside the vehicle.

Waylon was properly restrained in the rear passenger seat of the Ford Escape when the collision happened. He survived the initial impact but suffered critical injuries. Emergency crews took him to Norton Children’s Hospital in Louisville, where he later died from injuries caused by the crash.

Investigators believe alcohol consumption by Middlebrooks likely played a role in the collision. That finding has not yet been made final. Toxicology results were still pending, and the investigation into what led Middlebrooks to enter the southbound lanes remained underway.

The crash shut down all southbound lanes of I-65 near Seymour for about four hours while investigators examined the wreck and crews cleared the roadway. Several agencies assisted, including the Indiana State Police Crash Reconstruction Team, Seymour police, the Jackson County Sheriff’s Department, emergency medical crews, local fire departments and the Jackson County Coroner’s Office.

The deaths of Alyssa and Waylon turned a late-night highway crash into the loss of a mother and her young child from Scottsburg. Authorities have not released any further details about the family beyond the information connected to the crash. The investigation remains open as police await toxicology results and work to establish the full circumstances.

  • Charleston, West Virginia: James McDaniel and Three Friends Die After Vehicle Goes Over Interstate Barrier

Charleston, West Virginia: James McDaniel and Three Friends Die After Vehicle Goes Over Interstate Barrier

James McDaniel, 20, Colten Garcia, 20, Kaden Ashby, 21, and Kaiden Radford, 21, were killed when the vehicle they were traveling in crashed near the I-77 and I-64 split in Charleston, West Virginia. All four were from Raleigh County, turning what had been a night with friends into a deadly crash that is now under investigation.

The collision happened late Friday night, Aug. 21, near Pennsylvania Avenue. Police said the vehicle was traveling on the interstate when it struck a concrete barrier and went over the elevated roadway. It then fell roughly 42 feet into the Martin Marietta gravel yard below.

Emergency crews responded to the crash, but there was nothing they could do to save the four occupants. McDaniel, Garcia, Ashby and Radford were all pronounced dead at the scene. Their ages made the loss especially difficult for the people who knew them, with each victim just 20 or 21 years old.

Police believe the group may have been returning from the Jason Aldean concert at the Charleston Coliseum and Convention Center. The crash site is close to the interstate split, and investigators said the vehicle could only have been on that section of the highway for a couple of minutes before it went over the wall.

The exact cause remains unknown. Charleston police are continuing their investigation and have not publicly determined whether a specific factor caused the driver to lose control. For now, investigators are focused on establishing exactly what happened before the vehicle hit the barrier.

The location itself is also getting renewed attention. Police said the deadly wreck was the third accident in that area within a week. It was also the second crash during that same period in which a vehicle went over the edge of the retaining wall. That has added to concerns about a stretch of interstate where serious crashes have happened before.

Transportation and police officials have been looking at possible safety measures in the area, while investigators continue examining the latest crash. Police have also urged drivers to slow down, stay focused and avoid distractions because several factors can contribute when vehicles leave the interstate.

But behind the road-safety questions are four families dealing with a much more personal loss. James McDaniel, Colten Garcia, Kaden Ashby and Kaiden Radford were young men from the same county whose lives ended in the same crash. Their deaths have left friends, relatives and others across Raleigh County mourning while Charleston police continue searching for answers.

  • James McDaniel, Charleston, West Virginia: Four Young Men Killed After Car Plunges From Interstate

James McDaniel, Charleston, West Virginia: Four Young Men Killed After Car Plunges From Interstate

James McDaniel, 20, Colten Garcia, 20, Kaden Ashby, 21, and Kaiden Radford, 21, were identified as the four young men killed in a crash in Charleston, West Virginia. The four were from Raleigh County and were traveling together when their vehicle left an elevated section of interstate near the I-77 and I-64 split.

The crash happened late Friday night, Aug. 21, near Pennsylvania Avenue. Police said the vehicle struck a concrete barrier on the interstate before going over the edge and falling into the Martin Marietta gravel yard below. The drop was about 42 feet.

All four men were pronounced dead at the scene. The violent crash left their families facing the sudden loss of four young people who were only 20 or 21 years old. Friends and others who knew them have been left trying to make sense of what happened during what was supposed to be an ordinary night out.

Police have said the four may have been leaving the Jason Aldean concert at the Charleston Coliseum and Convention Center. Because of where the crash happened, investigators believe they had been on the interstate for only a short time before the vehicle went over the retaining wall.

The Charleston Police Department is still investigating what caused the crash. Authorities have not announced a final explanation for why the vehicle struck the barrier and left the elevated roadway. Investigators are working to piece together the events leading up to the deadly impact.

The crash has also brought fresh attention to that section of interstate. Police said it was the third crash in the area within a week and the second during that period in which a vehicle went over the edge of the retaining wall. That history has raised new questions about what can be done to make the stretch safer.

For the families of McDaniel, Garcia, Ashby and Radford, though, the focus is much closer to home. Four lives were lost in a matter of seconds, and four families are now dealing with a loss that cannot be undone. The young men were not simply names in a police report. They were sons, brothers, friends and people with their own plans for the future.

As the investigation continues, their names remain at the center of the tragedy: James McDaniel, Colten Garcia, Kaden Ashby and Kaiden Radford. Their deaths have brought grief to Raleigh County and Charleston, while those who knew them are left holding on to memories of four young lives that ended far too soon.

  • Priscila, Delran, NJ: Alvares Pereira Dos Santos Found 20 Feet Offshore After River Rescue

Priscila, Delran, NJ: Alvares Pereira Dos Santos Found 20 Feet Offshore After River Rescue

Priscila Alvares Pereira Dos Santos, 33, was found dead Tuesday morning in the Delaware River after she disappeared while helping a young girl who was struggling in the water at Amico Island Park. Her body was located about 20 feet from the beach, ending an overnight search that began after she went underwater during the rescue.

The incident started shortly before 4 p.m. Monday, Aug. 24, when Delran police were called to the park at 81 Norman Ave. for reports of two possible drowning victims. Investigators determined that Priscila had entered the river to help the girl.

Priscila reached the child and pushed her toward the shore. People standing nearby were then able to pull the girl from the water. The rescue effort had saved the child from the immediate danger, but Priscila went underwater during the attempt and failed to resurface.

Emergency crews began searching the river soon after. The operation involved Delran police and firefighters, the Delran Emergency Squad, New Jersey State Police, the Camden County dive team and Philadelphia police. The search centered around the area where Rancocas Creek meets the Delaware River.

The conditions in the river also became part of the search challenge. Delran Fire Department Deputy Chief Kevin Peak said the tide was moving out when reports came in that Priscila was submerged. He noted that river currents can pull people under in much the same way strong currents can affect swimmers in the ocean.

Search crews kept working into Monday night, but the operation was suspended around 9:45 p.m. It resumed about 6:30 a.m. Tuesday as a recovery effort. Drones were deployed at about 6:45 a.m., giving crews another way to scan the water and nearby areas.

About an hour and a half later, at roughly 8:15 a.m., Priscila’s body was located about 20 feet off the beach at Amico Island Park. Authorities then recovered her from the water. The park remained closed Tuesday as police and other agencies continued their work.

Police said the death was not believed to be suspicious. The investigation is still underway, and the Burlington County Medical Examiner’s Office will conduct an autopsy. Authorities have not released an official cause of death.

What happened at Amico Island Park was a rescue that turned tragic in a matter of moments. A girl who had been struggling in the Delaware River was brought safely to shore after Priscila pushed her toward waiting bystanders. Priscila did not make it back.

The search that followed stretched through the night and into Tuesday morning before crews found her. For those who knew Priscila, the circumstances of her death leave behind both grief and the knowledge that her final actions were focused on getting a child out of danger.
